How Does Optimal Health Coaching™ Facilitate Lifestyle Behavior Change?
- Each participant is provided regular opportunities to communicate with an Optimal Health Coach™, building a relationship of trust, support, and encouragement.
- Each Health Coaching session consists of open dialogue about the individual's identified modifiable health risks, overcoming barriers, and goal setting. The individual is provided targeted health recommendations and education, guidance, and positive feedback.
- Participants work with their Health Coach to create their personalized Optimal Health Action Plan™. These plans are designed to provide a structured framework with timelines, progress indicators and outcome measures targeted to achievement of the individual's desired health goals.
- Health Coaches also refer participants to available resources within the company and community such as Disease Management, Employee Assistance Programs, benefit incentives, community programs, and provider services. Our referrals and support of existing employer programs ensures a cohesive and collaborative wellness program is presented to participants.
How Is Health Coaching Progress Documented?
- Health Coaches document, track, monitor and measure outcome data and provide quarterly aggregate reports on pertinent trends for each company.
